As a member of an interdisciplinary team which includes physicians, physician assistants and nurse practitioners, the Primary Care Behavioral Health Clinician (PCBHC) is responsible for providing consultation, brief cognitive behavioral assessment and treatment interventions to a primary care population including patients with identified behavioral health needs. Patients may have an acute psychological response to a medical event and to patients with emerging medical or chronic medical conditions. The clinician will provide care management and triage services for patients who need to be or are involved with the community health team. The Clinician will work with other mental health providers to arrange services for practice patients who need long-term psychotherapy. The process of care will often feature a clinical handoff from physician to clinician. This is an evidence-based, population based model of care. Clinicians will be expected to have working knowledge and skills in working with individuals, couples and families with the full spectrum of mental health and substance use disorders.


Master's degree in an social work. Appropriate State of Vermont licensure required.


Minimum two years experience in a human services organization, health care setting or managed care system required.
