Position Overview:


Sutter Gould Medical Foundation (SGMF)is a non-profit, multi-specialty health care organization affiliated with Sutter Health. Based in Modesto, SGMF provides quality medical care, health education and research activities in a five-county area. SGMF's medical staff includes family practitioners, internists and pediatricians, as well as a range of specialist physicians. Services include imaging, nuclear medicine, a health library, health education programs, occupational medicine, diabetes education, nutrition counseling, infertility counseling, and weight management.

POSITION SUMMARY:
Under the direction of the Manager of Community Health Education in collaboration with RD, CDE's and other staff, this position is responsible for planning and facilitating self-management training, and maintaining ADA recognition for Sutter Gould Diabetes Education Program. This position is responsible for overseeing the Continuous Glucose Monitoring System and Insulin Pump program. Other responsibilities include participating in community events and teaching health-related classes. The Health Educator RN, CDE and Health Educators, RD, CDE are cross trained in all aspects of diabetes education. This position will require traveling to SGMF Care Centers including Stockton.

Qualifications:


MINIMUM POSITION REQUIREMENTS:
  • Associate's degree in Nursing is required

  • Registered Nurse License in California.

  • Current BLS for Healthcare Providers cared required.

  • Must have previous experience working in an outpatient healthcare environment.

  • Experience developing, implementing and presenting education programs.

  • Experience in the care of people with chronic disease is essential.

  • Must have had a successful experience working with diverse populations.

Skills & Knowledge:
  • Knowledge of learning theory and instructional skills, education theory and techniques.

  • Knowledge of how to develop learning goals and objectives.

  • Must have a working knowledge of computers.

  • Must have knowledge of current diabetes treatment recommendations.

  • Ability to read, write and effectively communicate verbally in English.

  • Public speaking skills are needed.

  • Must be able to work independently and manage time efficiently in a varied work environment.

  • Excellent customer skills are needed.

  • Critical thinking skills are needed.

  • Ability to assess, develop, prepare and implement education for individuals and in a group setting.

  • Ability to evaluate and report outcomes.

  • Ability to use a personal computer and software applications to create overhead presentations.

  • Be familiar with audiovisual equipment.

  • Needs to work well with children and adults of diverse backgrounds.

  • Ability to develop a good rapport with physicians.



Preferred Qualifications:
  • Bachelor's degree in Nursing.

  • A Certification as a Diabetic Educator (CDE) is preferred.

  • Second language (Spanish) is desirable.
